Since you ordered imprinting on your cards, please complete
the information requested in the form below and submit.

IMPORTANT
We will be sending you a PDF Proof within 24 - 48 hours (Monday-Friday) of receiving your info
to show you how your imprint will look on the card. If you do not receive your proof, please check
your SPAM or JUNK eMAIL folder and remove us from your "blocked" eMail list so that we can
communicate with you. If it's not in your SPAM or JUNK eMAIL folder, please contact us as soon as
possible to let us know (allow at least 48 hours, Monday-Friday). The form below requests a phone
number that we can contact you at if we don't hear back from you within a reasonable amount of time.

NO CARDS WILL BE IMPRINTED UNTIL WE GET YOUR OK!

PLEASE CHECK YOUR PROOF CAREFULLY! ... Imprinted cards are non-refundable!
You will be charged accordingly for ANY changes after your final OK if your order has
already been imprinted and/or shipped (even if it was our typo).
